A virtual call from your healthcare team first; with an AllCare Medical Assistant and then with your Provider. A risk assessment that is geared towards wellness for our Medicare patients. The questionnaire is designed to accurately review the patients’ health and wellness needs. At the end of the assessment, your Provider will be able to create a plan of care that aides in governing your overall health for the year. You’ll be provided with tools and local resources to assist you with your everyday living and health. An option to review your social needs, such as your living environment, access to resources and family circumstances. Your provider will discuss how these social needs impact on your health and how we can help you to improve them. Follow-up appointments and continued care can be scheduled at this time as well.

The earlier we schedule the more time throughout the year you will have to address and treat any possible health issues or new care plans for the future. Completing the assessment prior to your appointment will allow more time for your actual appointment. You will be given personal time with your provider to discuss your health concerns, progression and given information on how to build a healthy lifestyle for the year. The visit is for our Medicare recipients and will not cost you anything. It is absolutely free!

Yes, it is very different! The AWV appointment is only a virtual visit, being in the office is not a requirement. This is more of a preventative health assessment and wellness visit. Your provider will review a list of questions that are geared towards disease prevention and your personal healthcare goals. The questionnaire will be sent to you prior to the virtual appointment, to allow more time to address your healthcare needs and review your responses. No lab work and physical contact for the AWV unlike the annual Physical exam.
